Where to Apply for Child Health Plus and Family Health Plus

Child Health Plus A (CHPA)
CHPA is an insurance program for children under the age of 19 who are eligible for Medicaid coverage, including working families whose income is higher than the regular Medicaid income levels. Eligible children receive a Medicaid benefit card and are entitled to all medical services covered by Medicaid. There is no cost or monthly insurance premium. Most eligible children will also be enrolled into a health plan.

Child Health Plus B (CHPB)
CHPB is a state insurance program for children under the age of 19 who are not eligible for Medicaid. Parents choose the health plan their children are enrolled in. Once enrolled, children receive a health plan identification card and are entitled to a range of services covered by that health plan. Depending upon the families’ income, there may be a low monthly insurance premium.

Family Health Plus (FHP)
FHP is a state insurance program that serves single adults, childless couples and adults in families who are not eligible for Medicaid. Eligible persons are enrolled directly into the health plan and receive a health plan identification card. Enrollees receive all their health services through the health plan.

Medicaid and the Prenatal Care Assistance Program (PCAP)
Medicaid is the major federal/state program that provides health insurance for individuals and their families, particularly children, pregnant women (who can also apply for Medicaid through a PCAP provider), the elderly and the disabled. Eligible individuals receive a Medicaid benefit card. Most Medicaid-eligible persons will also be enrolled into a health plan. Persons already receiving Medicaid can call New York Medicaid CHOICE at 1-800-505-5678 about Medicaid Managed Care. Persons interested in applying for Medicaid or for coverage through PCAP can call HealthStat at 1-888-692-6116 for the location of the nearest community HealthStat office.

The charts below show the amount of monthly income (before taxes) at which you can get health insurance, effective January 2004. Income levels change annually. Also, depending upon age and health conditions, individuals within the same family may qualify for different programs.

Sudden Infant Death Syndrome (SIDS) is the leading cause of post-neonatal mortality, accounting for approximately 28 percent of such deaths. SIDS rates vary widely by race and et...
